Liability Waiver

I understand that while it is up to my discretion to choose to share my email under "Subscriptions", opting out of any email permissions may prevent Yoga Shakti from informing me of important studio information, class changes, class confirmation links and any other crucial & relevant information.  I agree that Yoga Shakti is not liable for any negative consequences such missing information may cause and that I am able to change the email preference settings in my profile at any time. 

 

I am aware that Yoga Shakti is here to serve me by sharing knowledge of Yoga, Meditation and Health. I hereby waive and release any claim that I might have at any time for any discomfort or injury be it physical, mental, emotional or of any sort against Yoga Shakti or any person or entity in any way involved therewith, including without limitation, its principals, instructors, employees, agents and representatives.

I understand that I must judge my own capabilities with respect to practicing Yoga or any other physical discipline, or Meditation at Yoga Shakti. By my participation in classes or activities at Yoga Shakti, I agree to take full responsibility for not exceeding my limits in the practices taught, for selecting the appropriate level of classes, and for any injury which might occur during the practice. I acknowledge that it is my responsibility to seek medical advice & ascertain that there is no medical reason to prevent my participation in classes at Yoga Shakti. I also acknowledge that it is my responsibility to inform the instructor, when I begin a class, of any condition that might affect my ability to participate and to inform the instructor immediately if any injury occurs during class.

I understand that, during classes, instructors may physically adjust students’ form when doing postures and movements. If I do not want such physical adjustments, I will so inform the instructor at each class I attend. I also understand that it is my responsibility to inform the instructor when an adjustment has gone as far as I desire at that time.

I understand and agree that Yoga Shakti is not responsible for the safekeeping of my personal belongings while I attend class.

 

Memberships and Class Series

All membership will auto renew every month on the monthly anniversary date.  Any unused classes at the end of the month will not be carried over to the following month. All promotion or discounts on memberships will auto renew into an unlimited monthly membership. Memberships must be cancelled 1 week (7 days) before the renewal date.

Class Series, unused classes will expire within 6 months of purchase. A one time three month extension can be purchased for 25% of the class series price.

In-Person Memberships and Series can be used for In-Person and Live-Stream (at class time) classes.

Due to the nature of Yoga Shakti's service based business, all sales are final with no exceptions.

Assumption of Risk and Waiver of Liability 

Yoga Shakti Studio (“The Studio”) has put in place preventative measures to reduce the spread of any respiratory illness, including COVID-19; however, The Studio cannot guarantee that you will not become infected with any illness. Attending classes could increase your risk of contracting an illness; there is risk involved at any public gathering.

By registering for classes you acknowledge and agree to the following:

By attending classes in-person, I voluntarily assume any risk that I may be exposed to or infected with an illness which may result in significant health consequences. I understand the risks and accept sole responsibility for any injury, illness, damage, loss, liability or expense of any kind that my ensue in connection with my attendance of classes/events at Yoga Shakti, in-person or virtual I hereby release, covenant not to sue, and hold harmless Yoga Shakti Studio, its employees, officers, agents, representatives and Landlord of any and all claims based on the actions or omissions of said entities.  


I voluntarily assume any risk that I may be exposed to or infected with an illness which may result in significant health consequences.  I understand the risks and accept sole responsibility for any injury, illness, damage, loss, liability or expense of any kind that my ensue in connection with my attendance of classes/events at Yoga Shakti. I hereby release, covenant not to sue, and hold harmless Yoga Shakti Studio, its employees, officers, agents, representatives and Landlord of any and all claims based on the actions or omissions of said entities. 

Video Release

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, photographic, audio or video recordings may be used for the purposes of on-line (livestream) classes, workshops and programs.

By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public live stream setting.

This release applies to photographic, audio or video recordings collected as part of my attendance of live classes at Yoga Shakti Studio. By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational or business purposes.

By purchasing, registering, and or participating in classes you agree that you have carefully read the release, and fully understand and agree to the above.
